MIPSS Enrollment Requirements for New & Transferee Students

Students with Special Education Needs

• Evaluation report from the following:
  • Neurodevelopmental Pediatrician
  • Speech Therapist (if available)
  • Occupational Therapist (if available)
  • Physical Therapist (if available)
• Report Card / Teacher's Evaluation
• PSA Birth Certificate
• Baptismal Certificate
• 1x1 Picture (white background) with collar
• Marriage Certificate of parent (if married)
• Certification of Eligibility from DepEd / ACR (Alien Certificate of Registration) if the enrollee is a non-Filipino citizen

For Nursery - Kinder

Age Requirements (as of August 30):

• Nursery: 4 years old
• Kinder: 5 years old

Requirements:

• PSA Birth Certificate
• Baptismal Certificate
• 1x1 Picture (white background) with collar
• Marriage Certificate of parent (if married)
• Certification of Eligibility from DepEd / ACR (Alien Certificate of Registration) if the enrollee is a non-Filipino citizen

For Grades 1–6

• PSA Birth Certificate
• Baptismal Certificate
• Report Card with Learning Reference Number (LRN) or Certificate of Completion
• SF10 (Form 137)
• Certificate of Good Moral Character
• 1x1 Picture (white background) with collar
• Marriage Certificate of parent (if married)
• Certification of Eligibility from DepEd / ACR (Alien Certificate of Registration) if the enrollee is a non-Filipino citizen
• Early Childhood Care and Development (ECCD) Certificate (for incoming Grade 1; obtainable from the child's previous school)

For Grades 7–12

• PSA Birth Certificate
• Baptismal Certificate
• Report Card with Learning Reference Number (LRN)
• Permanent Record (Form 137)
• Certificate of Good Moral Character
• ESC or QVR Certificate
• 1x1 Picture (white background) with collar
• Marriage Certificate of parent (if married)
• Certification of Eligibility from DepEd / ACR (Alien Certificate of Registration) if the enrollee is a non-Filipino citizen

For Grade 7 ESC Applicants

• ESC Application Form: Download Here
• Barangay Certificate of the student
• Latest Income Tax Return (ITR) for the previous year, or Certificate of Tax Exemption, or Municipal Certification of Unemployment of Parents/Legal Guardian
